I also offer virtual appointments via secure video platform to patients located in the states of New York, New Jersey, Connecticut, Maryland, and Florida.

I am an out-of-network provider, which means I do not work directly with any insurance company. Patients pay me directly for my services and then, if their insurance plan includes out-of-network benefits, submit a claim for reimbursement to their insurance company. I provide patients with a monthly superbill which contains the information needed to submit such a claim.
If you are uncertain about your out-of-network benefits, these are the questions I recommend exploring with your insurance company to get a sense of what reimbursement you can expect:
Does your insurance plan have out-of-network coverage for outpatient psychotherapy?
Does your insurance plan have an annual out-of-network deductible you need to meet before you will be reimbursed? If yes, what is the annual out-of-network deductible, and what percentage of it has already been met for this calendar year?
What does the insurance company consider the “reasonable and customary fee” for this service for this location? Services are identified by “CPT codes.” The CPT code for 45-minute individual therapy sessions is 90834. The CPT code for couples and family therapy is 90847. Your insurance company may also want Dr. Bermingham’s National Provider ID (NPI), which is 1992477434 and/or Dr. Bermingham’s zip code, which is 11242.
